1. Any finished scripts/software out there that can be used for this? Preferably open source/php that can be modified. I guess the ringtones are stored into a database and then retrieved from there?

2. Where to get all the ringtones? I'm not going to create them myself. Some kind of licenser?

3. Some kind of SMS service in USA? Customer sends SMS to website, who sends back the ringtone to the cellular.
